    your specific coverage details. When you contact your insurance
    company, let them know that you are inquiring about "outpatient
    mental health" benefits. Dr. Bolton suggests that you ask your
    insurance company several questions before scheduling an
    appointment, such as:

Does my coverage require that I get a pre-authorization  
before I schedule an appointment?

How many outpatient sessions per year am I covered for?

How much is my deductible? Does it apply for these types
           of services? If so, how much of my deductible have I met
           so far this year?

What is my co-payment or co-insurance per session?            
           (A co-payment is a set dollar amount. A co-insurance is a   
           percent of the fee or contracted rate).

Is Dr. Jonina Bolton an in-network provider for my insurance
company?

Do I have any "out-of-network" benefits? (In case you want
           to receive services from Dr. Bolton in the event that she is
           not an in-network provider for your insurance company.)

    Dr. Bolton accepts both self-pay and insurance-pay clients. If you are utilizing insurance benefits, then you are responsible for paying
    any deductible, co-payment or co-insurance as required by your specific policy. If you are self-pay, then you are responsible for
    paying the full fee for services.

    Payment Options
    Most individuals would like the benefit of working with their therapist for as long as is needed to accomplish their personal goals.
    The reality is that the manner in which you pay for your treatment may affect the confidentiality and extent of care that you receive.
    If you pay all the fees for your services (self-pay), then you and Dr. Bolton will determine the type of services that will benefit you the
    most and the length of time that you will remain in treatment.  If you utilize insurance benefits, then your insurance company will
    require Dr. Bolton to submit a diagnosis and description of treatment services and may exercise a considerable amount of control
    over the type of services that you receive, as well as the number of treatment sessions that will be covered. Insurance companies also
    reserve the right to inspect client records and will be able to access information about session content.
        As the result of the issues described above, many people decide that the direct payment (self-pay) option, without the involvement
    of an insurance company, is best suited for their needs. The self-pay option allows for a higher degree of confidentiality while providing

    Payment is due at the time services are rendered. Dr. Bolton accepts cash, personal checks and payments through bill pay services
    with electronic banking.  Credit cards and debit cards are not accepted.
